To start with, it’s important to understand that this illness evolves all the time and therefore the symptoms explained below can change and even intensify with time. Another factor to take into account is the individual themselves each individual is unique which can generate some variations in the clinical analysis… On the other hand, there are some characteristic which are common to everyone who is affected by this illness

Here are the details of it:

The predominant pain in this illness is a burning feeling. It is either unilateral or bilateral and appears mainly in the rectal, perineal, urethral and/or genital organs (clitoris, vagina, penis, testicles …. In the case of severe attack, the pain can also spread towards the thighs. Patients describe a real sensation of intense burning.

Other symptoms also exist such as the feeling of a foreign object in the rectum (tennis ball),intense* electric shocks (pins and needles) deep pinching in the rectum as well as twisting in the perineal area. The pain is permanent, daily and worse in the sitting position*. They generally start in the morning when daily life begins (breakfast time) and increase during the day to reach unbearable level in the evening. The pain gets better when you lie down.
* The pain is very often unbearable, reaching 10 on the VAS (Visual Analog Scale) which measure the pain level experienced by patients, from 0 to 10, 10 being the maximum pain level you can imagine/define.

That is for the “typical” form of the illness, but as we explained in the introduction, associated problems very often occur from the beginning of the illness and even afterwards.

At the top of the list are urinary and fecal problems. They are frequent and can often aggravate the symptoms described above. They are burning sensation when urinating, an increase in urinary frequency as well as some sphincter troubles such as dysurie (urination hesitation), difficulty expulsing the last spray of urine and small post- urination leak. There is also an urge to urinate even when the bladder is almost empty. There are pains linked to defecation followed by diarrhoea and/or constipation. The feelings of discomfort in the rectum don’t improve the situation and encourage the patient to multiply his movements that result in the aggravation of the initial symptoms. These anomalies generate some serious intestinal problems causing pains in the lower abdomen that are qualified by the medical profession as irritable bowel syndrome.
Sexual troubles also occurred very often and can take different forms. For men, ejaculation is painful and make things worse. The erection is less firm and sometimes doesn’t happen. The control of ejaculation is difficult and the expulsion of the sperm doesn’t happen properly. For women, the problems are similar. Pains during intercourse, absence of pleasure, clitoris and vulva over sensitive, discomfort during penetration...
In plain language, the libido is highly affected by all these symptoms and very often patients will stop all sexual activity (source of pain).
